This will depend on your hemoglobin A1c and your doctor's preferences. The treatment guidelines recommend starting people on one oral medication along with diet and exercise if their A1c is less than 7 percent. If the A1c is between 7 percent and 8.5 percent, one medication or a combination of medications will be started.
 
People with an A1c higher than 8.5 percent will benefit from two medications or insulin. Medications lower the A1c faster than diet and exercise alone. Deciding when diet and exercise are not enough (and when medications are needed) will be up to your healthcare provider. If you have pre-diabetes, diet and exercise would be a good option to help prevent diabetes
